CNS Hospice Care
Hospice isn't a place— it's a comprehensive approach to care. Providing emotional, physical and spiritual support for patients and their loved ones in the final phases of an illness, the hospice philosophy recognizes that the end of life can and should be dignified and comfortable.
Although anyone may request information about hospice care, a physician's order is requested to initiate services.

CARE WHERE IT'S NEEDED MOST
CNS services are available wherever the patient resides – at home, in a nursing home or in an inpatient facility – anywhere in Chicago's western suburbs and beyond.
Experienced staff and depth of resources mean CNS can provide a scope of services that set us apart.
That care includes skilled nursing, expert pain and symptom management. The CNS hospice team works with the patient, their loved ones and the primary care physician to design a plan of care addressing medical, emotional and spiritual needs.

SUPPORT WHEN IT'S NEEDED MOST
Losing a loved one — whether a spouse, parent, child or friend — is a painful and often difficult passage in life. As longtime providers of end-of-life care, the staff and volunteers at CNS Home Health & Hospice understand that grief is a powerful force, one that can leave you feeling alone, uncertain and frightened. The bereavement program at CNS underscores the belief that grief needs to be seen, heard and acknowledged — and that it's an experience that is both intensely personal and at the same time universal. Mourning is a complex journey, but it's not one that you have to travel alone.
UNDERSTANDING, ACKNOWLEDGEMENT AND HEALING
CNS bereavement counselors help those who have experienced a loss to understand the normalcy of the many dimensions of grief, in an environment that encourages expression. Here, you will learn about the symptoms and cycles of the grieving process. By understanding your pain, you can come to terms with your loss and move forward with life.

ELIGIBILITY
Families who have received CNS hospice services are provided with bereavement care for up to 12 months after the death of a loved one. Involvement in the hospice program prior to your loss is not a prerequisite to participating in the bereavement support groups. One-on-one grief counseling and bereavement support groups are free to both family members of CNS hospice patients and to the community.
