Home Health
While caregivers of the past were generally family members or friends, Home Health care today is based on a professional health care team working toward a common goal.
Our Home Health care program and team of caregivers provide care to patients who cannot get to the doctor or an outpatient facility. Our nurses and other professionals visit as often as necessary to meet your medical needs and to help get you back to the highest level of independence as possible. Home Health care is provided by professional caregivers in the home, apartment or retirement facility under the direction of the patient’s physician.
Our Home Health services include special programs for individuals with needs in Cardiac, Wound, Behavioral Health, Telehealth and GAP (Geriatric Assistance Program).
Registered nurses offer a wide range of services:
- Medication use and safety instruction
- Self-testing procedure instruction such as blood sugar monitoring for diabetes
- Wound Management such as surgical wound dressing changes
- Cardiac treatment and education
- TeleHealth
- Signs and symptoms that alert you to contact your doctor
- Diet management
- Disease management
- Home Infusions IV therapy
Physical, occupational and speech therapists instruct in:
- Exercise programs and other appropriate skills to regain your strength of movement
- Verbal and swallowing skills
- Self-care such as feeding, dressing and other daily activities
- Your safety in the home environment
- Post-operative hip and knee replacement care
Medical social workers are available to assist with:
- Financial assistance applications
- Behavioral issues that can adversely affect health
- Community resources such as home-delivered meals or emergency alert systems
- Other family concerns
Home Health care professionals may also assist with personal needs such as bathing, feeding and light meal preparation on a limited basis. Please see our Private Duty services section for more information about our Bath Program.
Home Health care services may be paid for directly by the patient, through insurance coverage, or through other public or private sources.
