Hospice
Hospice is the model for providing compassionate, quality care at the end of life. Hospice strives for a pain-free, peaceful and dignified death. Caring, not curing, is the focus.
Managing pain and symptoms is an important part of hospice. So is providing emotional and spiritual support. Our goal is to meet the individual needs of each patient and family. We encourage family members to participate in developing and implementing the plan of care. Families can choose the clinical and support services that are important to them.
Hospice is provided in the patient’s home, in nursing homes, independent and assisted living facilities as well as hospitals.
Our hospice team includes registered nurses, social workers*, chaplains*, home health aides, volunteers*, a medical director, an attending physician and bereavement personnel*.
Admission to hospice is open to all, regardless of age, gender, religion, race and sexual orientation. Diagnosis is not restricted to cancer. Any patient who is terminally ill is fully eligible. Referral should be made as early as possible, up to six months prior to expected death. This way, the patient and family can receive the full benefit that hospice brings to help provide quality of life at the end of life.
Hospice is covered by Medicare, Medicaid and most private insurance plans. Self-pay is an option; however, no patient will be turned away due to inability to pay.
*A patient and/or family may, at any time, choose not to have the social worker, chaplain, home health aide, volunteer or bereavement personnel involved in their care.
