CNS Hospice is a not-for-profit,
Medicare-certified, JCAHO-accredited, state-licensed, full-service hospice program.
Interdisciplinary team visits address the comprehensive, holistic needs of both the
patient and family. Regularly scheduled visits include services from the interdisciplinary
team:

RN--for pain management and symptom control

Certified Nurse Assistant (CNA)--for assistance with personal care
-
Social Worker--for
emotional support and assistance in finding help through community resources
-
Chaplain--for spiritual support
-
Volunteer--for practical assistance and emotional support
-
Physical, Occupational and Speech Therapists--available as needed
-
Dietary
Consultation--available as needed
Other team members include the patient and family, the attending physician and the Hospice
Medical Director. Together, they make this difficult and sensitive time a little easier.
Also available:
24-hour, on-call RN--to answer questions, address concerns, and make emergency visits
Bereavement follow-up [link]--available free to residents of the CNS Home Health service
areas for 13 months. Involvement in hospice care prior to the loss is not a requirement.
CNS Hospice serves patients in DuPage, Will, Kane, McHenry and western Cook counties.
Patients receive care in their own home, in hospitals, or in over 30 nursing homes
throughout the service area.
When short-term hospitalization is required for pain control or symptom management, CNS
Hospice has agreements with most hospitals in the area.
Requesting Hospice Services
After curative and life-prolonging measures cease (prognosis of six months or less),
Hospice can step in. Patients and families may start experiencing a sense of isolation and
fear of abandonment.
The sooner a referral is made, the more time Hospice staff has to work with the patient
and family to live their remaining days in comfort and dignity. |
