CNS Home Health & Hospice brings a comprehensive line of health care services home to patients when and where they need it.  We are a not-for-profit agency, Medicare certified, state licensed and accredited by the Joint Commission on Accreditation of Healthcare Organizations.  We offer:

Skilled Nursing:  Provider-prescribed skilled nursing, plus a wide array of disease-specific programs, including Cardiac Care, Wound Care, and Behavioral Health

Rehabilitation Services:  Occupational, Physical and Speech Therapy; Music Therapy and Reflexology

Caring Choices:   Private duty RNs, LPNs, certified nurses’ aides and homemakers assisting patients with medical, home and personal care needs

Palliative Care:  Relief of pain, symptoms and stress from a serious illness, appropriate at any point in a medically complex illness;

Hospice Care:  Interdisciplinary team providing physical, spiritual and emotional care for patient and family members facing a life-limiting illness

IV and Infusion Services:  Antibiotics, Hydration, Pain Management, Chemotherapy, Blood Products, and Nutrition;

Home Medical Equipment:  Wide variety of respiratory care and medical equipment with 24-hour, 7 day a week service

Parish Nursing:  In partnership with local congregations, this program address the health and spiritual well being of faith community members. 

Bereavement Programs:  Community-wide individual counseling and support groups for a range of needs for anyone who has experienced a loss

Speakers Bureau:  Wide variety of programs on aging, hospice and palliative care, parish nursing, nutrition, living will, specific clinical programs and more
