Our professional staff has combined experience of more than 300 years within their areas of expertise including:

• Masters prepared Behavioral Health Nursing with expertise in depression screening and geriatric disease management.
• Infusion certified Registered Nurses to provide chemotherapy in the home, IV antibiotics, blood transfusions and other specialty drug regimens.
• Our cardiac team with advanced training and education are skilled in the management of congestive heart failure, post surgical cardiac bypass monitoring and the use of advanced telehealth to monitor patient well-being seven days a week.
• Our Physical Therapy team has advanced training in care of the post hip or knee replacment as well as specialized treatments such as anodyne therapy and treatment for lymph edema.
• The Palliative Care team offers exceptional multi-discipline services that include Palliative Care Board Certified Physician services, Advanced Practice Nursing, Masters prepared Social Workers and other specialists to meet the needs of patients receiving curative treatments such as radiation or chemotherapy on an outpatient basis.
• The Hospice team exemplifies compassion and caring toward end of life. Registered Nurses are all hospice certified, as well as two hospice team physicians who are available for consultation and communication with your primary physician of choice.
• Bereavement services are community wide and available with or without a prior relationship with CNS Home Health & Hospice.
