CNS
Home Health offers the following services:
- Skilled Nursing Care--RNs coordinate all patient care and
follow physician-prescribed treatments, including, catheter care, medication management,
ventilator and ostomy care. Services include patient and family education to help patients
achieve independence.
- Psychiatric Nursing--Patients receive comprehensive,
multi-disciplinary, at-home treatment that helps them function as independently as
possible. These services provide continuity of care between hospital and outpatient
treatment; and a possible alternative to hospitalization. CNS psychiatric nurses
coordinate multi-disciplinary services, including social services, occupational/physical
therapy, home health aides and others.
- High-Tech Nursing and IV solutions--RNs provide IV therapies,
including chemotherapy, fluid replacement, antibiotic, pain management and
hyperalimentation.
- Home Health Aides--(Also known as Certified Nurses Aides)
Aides help with basic nursing care, medication administration and personal care needs,
including bathing, shaving, shampooing, dressing and grooming.
- Medical Social Workers--Social workers provide patients and
families with assessment and options for additional services, plus planning and adapting
to present and future care needs.
- Physical, Occupational and Speech Therapies--Certified
therapists provide care to patients to achieve optimal independent functioning.
- Hospice--Interdisciplinary team visits address the
comprehensive, holistic needs of both the patient and family. Team includes hospice nurse,
home health aide, social worker, chaplain and therapists. Bereavement services and
counseling is also available.
- Private Duty Nursing--RNs, CNAs, LPNs and Homemakers assist
patients and families with medical and additional home care needs.
- Bath Program--Caregivers provide bathing and grooming
services, plus basic physical assessments for patients.
