CNS Home Health & Hospice is a not-for-profit agency which delivers a comprehensive, full-service home care program to assist patients with acute healthcare needs and their families.

- Hospice Care for uninsured or underinsured terminally ill patients.
- Palliative Care for those with life-threatening illnesses who are still seeking treatment.
- Home Health Care for uninsured or underinsured patients.
- Bereavement Support for those coping with the loss of a loved one.
- Geriatric Assistance for low-income, socially isolated, chronically ill elderly.
- Parish Nurse Program to assist churches with support for their parish nurses

Donate Goods and Services
"Birthday in a Bag"
Throughout the year, many of our hospice patients and Geriatric Assistance Patients celebrate milestone birthdays. We are looking for groups of families, scout troops, students, etc. to take on a project and create "Birthday in a bag" treats for our hospice patients. You are also welcome to participate in this project by simply donating items.
Baking Up Nice Surprises (BUNS)
Twice each year in the Spring and Fall volunteers are asked to bake “goodies” for our hospice patients and their family and patients in our Geriatric Assistance Program. The baked goods are packed in bags decorated by a local school and delivered by our staff.
